All you have to do is be an active patient who comes in for a scheduled visit during July and/or August and then fills out an in-office entry form to be entered into our summer raffle to win our GRAND PRIZE!

(Any online entries, please submit (via email or direct message) a screenshot of your entry with the patient’s name (that you want entered into the raffle).
Contest details: A patient with multiple appointments can enter at each visit. Blackout dates apply (including holidays). By entering the raffle, you agree to have us repost any pictures or videos.
